Care Coordinator

Location: Swindon

Salary £16,000 - £20,000/ year 

Hours: 37.5/ week (1 weekend in 3 or 4 depending on staffing level)

Paid overtime + excellent benefits 

Looking for:

  • The Ideal candidate will have come from a care background
  • Someone who is looking to progress in to a supervisory role or already has supervisory experience
  • You will be managing 10 carers daily and therefore it is important that you are an organised leader & team player by nature.
  • Qualifications in care are desirable.
  • You must have a good telephone manner and be able to remain calm under pressure.
  •  A flexible approach to work and working hours is essential.
  •  You maybe required to jump in and help from time to time on rounds, therefore someone who is willing to go above and beyond will be suited to this role.  
  • Someone who can write rotas weekly and good at administrative tasks
  • Has experience in caring for venerable adults
  • Someone who wants to progress into an office-based role

 

Main Tasks:

  • To be customer focused and deliver an outstanding customer experience
  • Identifying potential customers for new business, making initial contact and promoting the services of the company.
  • Supporting, maintaining and developing relationships with existing customers.
  • To focus on Quality Assurance and ensure that the Quality Assurance policy and procedures are adhered to.
  • To ensure that care services are delivered to customers in accordance with the agreed care package.
  • To be part of the team which ensure all office procedures (where applicable) conform to National Care Standards as indicated by Care Quality Commission
  • To be part of the team that ensures compliance with Care Quality Commission regulations and achieves high grading with the external audit and inspection process.
  • To process bookings and enquiries made from the company website in a timely and accurate manner.
  • To report directly to and provide support to the Healthcare Mangers as necessary.
  • To develop close professional working relationships with Social Work staff and allied health professionals
  • To manage Care Workers in accordance with the company Standards of Service and maintain close working relationships with staff ensuring good working practice and staff development are encouraged
  • To manage the change in service requirements and follow up on corresponding change in carer workload and rotas.
  • To be part of a professional and competent rotational on call service covering evenings, weekends and public holidays. This may vary in frequency to meet the demands of the service. 
  • To ensure accurate database input of all information relating to Community Care Clients and Workers. 
  • Maintain excellent standards of departmental administration.
  • Ensure all enquiries and complaints are dealt with immediately and appropriately in accordance with the company Policies and Procedures.
  • To take telephone calls promptly and effectively, imparting all relevant information to appropriate office staff.
  • To ensure all communications are given and received effectively.
  • To promote a high level of health and safety awareness and commitment, and to be aware of the provisions as detailed in the Company Health & Safety policy.
